Getting Ready for Your Expedition
Please download and print the following forms. Parents/Guardians of expedition participants should fill out the OTC Medication Permission form, and you should have your child’s physician fill out the physical form before returning both to us by mail at 4472 Basin Harbor Road, Vergennes VT 05491 or via email at LauraC@lcmm.org. This form is due back to us 30 days before your course starts. If you are unable to send it to us within that time, please contact us.
